Hospice Program
Hospice is care which is delivered at home or in a home- like setting to persons in the final phase of any terminal illness. The goal of the Hospice Program is to allow clients to live with dignity, alert and as comfortable as possible, while involving families and loved ones in the care- giving. An interdisciplinary team provides compassionate care, including skilled nursing, social work services and pain and symptom management. Patients and families receive support during the illness and follow up of the families continue for 13 months after the death of a loved one.
